The foundation of any successful community health needs assessment (CHNA) and community health improvement plan (CHIP) is the active and ongoing involvement of the community. The Boston CHNA-CHIP Collaborative will provide many opportunities for the active engagement of residents of Boston’s neighborhoods. These are opportunities to provide the perspective of the community, but also help identify priorities and shape implementation plans to address those issues.

You are encouraged to consider joining one of the Collaborative’s workgroups.
