The CHNA-CHIP Collaborative brings together diverse individuals and organizations across sectors to build a shared vision of health for our city, and to promote healthy, safe, and thriving communities through developing and implementing strategies to address the barriers to health. The Community Health Improvement Plan (CHIP) process offers an opportunity to share best practices and resources, identify gaps and opportunities for strengthened collaboration, and to take action on new ideas.
